Participant comments regarding your IM experience are important to us. Feedback on facility or staff issues as well as stories of memorable moments are welcome. Please use our Online Comment Form to direct your comments to a particular staff member. We'll respond, as needed, as soon as we can.
Injured Participants & Athletic Training Referrals
Injured during an IM sports contest or at an IM facility? First, we recommend that you get the injury checked out by a medical professional. You are also welcome to be seen by the department's athletic training staff located in the FSU Health and Wellness Center, on the 2nd level next door to the Fitness & Movement Clinic. If you received a yellow Athletic Training Referral card when your reported your injury to our staff, you will receive priority over other walk-in patients.

To begin the reinstatement procedure, ejected participants should submit a statement using our Ejected Participant Response Form . Next, you'll set-up and attend an appointment to discuss the incident with an administrator who will rule on any further action and your reinstatement for further IM play.
Protesting captains should complete our Team Captain Protest Form with information (including specific rules) related to your protest. The administrative staff will review and rule on your situation in a timely manner.
Eligibility Appeal Form
Persons who do not meet the eligibility requirements to play Intramural Sports and who would like to petition the IM Staff for a waiver should complete our Participant Eligibility Appeal Form . The administrative staff will review and rule on your situation in a timely manner.
