| End of the Year Report | Online | Due at the end of the spring semester every year |
| Officer Delegation Worksheet | Online | A detailed sheet documenting officer responsibilities and delegations. |
| Practice Request Form | Online | For requesting practice space each semester |
| Practice Cancellation Form | Online | For cancelling requested practice space on campus. Must be completed by 12pm noon day of. |
| Visiting Team Waiver | Online | Teams hosting events must get this information from the visiting team |
| Tomahawk Point Submission Form | Online | Teams can fill this form out to receive Tomahawk Points for community service, leadership series, attendance at other club events, fundraising, etc. |
