Members: Complete Your Waiver Form Online
All current, active sport club members must complete the online Participation Statement and Informed Consent Form (waiver) prior to participating in any club activity including practices. The waiver is now online through Rec Connect, here on the Campus Rec web site. Here are some step-by-step sport club online waiver instructions to help guide you through the system. It's relatively easy, but we don't want you to miss a step! Note this is only for active members! If you are looking to join a club, contact the club first by visiting our Club Contacts page for contact information.
